The majority of isolated ASAT elevations in children reveal nothing serious. A follow-up after a few days of physical rest is often enough to normalize the result. Feedback on this point varies among practitioners, but most recommend a delay of two to three weeks before retesting.
Signals that justify immediate consultation
Some situations change the game and require prompt medical advice:
- A visible jaundice (yellowing) on the skin or the whites of the eyes, even if subtle.
- Dark urine associated with pale stools, which suggests a biliary obstruction.
- Unusual and persistent fatigue, prolonged refusal to eat, or unexplained weight loss.
- Simultaneous elevation of ALAT, GGT, and bilirubin, indicating a picture of real liver distress.
- A context of recent medication intake (certain antibiotics, antiepileptics, or anti-inflammatories can cause hepatic cytolysis).
An abnormal result isolated from a single blood test does not constitute a diagnosis. The physician reasons in trends: it is the persistence or worsening of the level, combined with clinical symptoms, that triggers further explorations (liver ultrasound, hepatitis serologies, in-depth muscular assessment).
The right reflex in the face of an unexpected result
Do not attempt to interpret the result alone by comparing it with adult norms found online. Forward the results to the prescribing physician remains the most effective approach. They have the complete clinical context: physical examination, vaccination history, ongoing treatments, and the child’s level of physical activity.
A control test conducted at rest, away from any effort and without interfering medication, provides a reliable picture. If ASAT normalizes, we close the case. If it remains elevated with ALAT still normal, the physician will explore the muscular or hematological pathway before considering a rare hepatic cause.
